About Adenomyosis

Adenomyosis is uterine tissue growing abnormally.

  • The body of the uterus consists of 3 layers
  • The outer skin or serosa
  • The middle thick layer of muscle called myometrium
  • The inner layer called endometrium
  • The endometrium grows in thickness during each menstrual cycle
  • If a pregnancy occurs it implants in the endometrium
  • If there is no pregnancy during the current cycle (Approximately 21-35 days with average 28 days), the endometrium is shed off with bleeding (Menses)
  • Adenomyosis is invasion of endometrium into the muscle layer (Myometrium)
  • Like normal endometrium it too is prone to growth and shedding cycles and bleeding within the myometrium

Symptoms

Symptoms of Adenomyosis

  • Symptoms are due to invasion of the myometrium by endometrium and bleeding along with inflammation and thickening of the uterine wall by fibrosis
  • The symptoms include severe pain during menses (Dysmenorrhea), chronic pain in abdomen, heavy menstrual bleeding (Menorrhagia) and feeling of heaviness in lower abdomen
  • Adenomyosis can also be associated with infertility

Imaging and Evaluation



Ultrasound

Ultrasound is sufficient to diagnose the disease .

MRI

MRI can be considered in case of uncertain diagnosis, to map the disease and rule out / evaluate pelvic endometriosis

Treatment

Adenomyosis treatment includes medications, surgery, and lifestyle changes.

Adenomyosis treatment options include pain management, hormone therapy, and in some cases, surgery like hysterectomy.

Woman’s Age

Treatment depends on the woman’s age and whether she desires childbearing

Pain killers and drugs

Mild symptoms can be managed with antispasmodics and pain killers and drugs designed to control bleeding

Oral contraceptive pills

If pregnancy is not desired, severe symptoms can be managed with hormonal agents such as oral contraceptive pills and progestogens

Surgery

Severe symptoms not responding to medical treatment require surgery

If the women desires conservation of the uterus, a wedge of the enlarged uterus is excised (Wedge resection of adenomyosis) or major chunk of uterine muscle removed along with endometrium (Adenomyometrectomy)
